If you are a new client, please download and fill out all 3 forms on this page and bring to your appointment with you.  Please note in Policies of Crescent Moon there are some changes:   The waiting room is now open so disregard what the form says about the waiting room being closed.  The mask mandate has been lifted for healthcare so masks are no longer required.  I do have masks, if you'd like me to wear one.  These are the only changes to the "Policies of Crescent Moon" intake form.
